If you're not sure what your performer paperwork should look like, start here. These are blank templates you can download, fill in, and use for your own shoots.
The templates
- Model Release / 2257 Agreement — the co-performer release agreement covering consent, age verification, and the producer's right to use the content.
- Sexual Activity Consent Agreement — records what each performer consented to, with space for stage name, legal name, director, production company, and site.
- Pre- and Post-Shoot Questionnaire — the on-camera questions to ask before and after a shoot, which creates a recorded record alongside the signed paperwork.
Click any name above to download it. All three are also attached at the bottom of this article.
How to use them
- Download the templates and fill in your own details where the brackets appear (producer name, production company, site).
- Have each performer complete and sign them before the shoot, alongside checking their photo ID.
- Record the questionnaire on camera if you're using it — that recording is part of your records.
- Upload the completed documents to the performer in your admin — see Compliance documents for performers (2257).
What you need on file for each performer
- A copy of a valid government-issued photo ID
- Selfie of performer holding ID
- A signed model release / consent form
- A signed 2257 form
Important
These templates are provided as a starting point, not legal advice. Requirements vary by where you and your performers are located, and by the content you produce. If you have any doubt about whether your paperwork is sufficient, speak to a lawyer — and contact support before publishing if you're unsure what we need.
Upload paperwork ahead of your planned publish date so there's time for review — see How long does 2257 document approval take?.
